Medical Harm happens when in spite of the many safety checks that healthcare systems have in place, errors my happen, which may cause avoidable harm to the patient. Patients also have a significant role in preventing this, but may not be aware of the same.

Many may have benefited by exceptional life- saving efforts of HCP (health care providers).
